Referrals to Choice Care come from all kinds of people and places – Social Workers, schools or colleges, families and people looking for support for themselves.

Whatever route, the process of finding the right support can seem complicated as it’s full of forms, steps and professionals to speak to. But at Choice, from the first ‘hello’ to moving in, we’re with you every step of the way – to make things feel a little easier and focus on what truly matters – you.

Let’s have a chat

All we need to get started are a few quick details. You can call, email or complete the Referrals Form to get in touch with our dedicated Referrals Team.

You might already have detailed notes or a care plan, or maybe you’re just starting to explore your options.

Either way, we’ll meet you wherever you are, listen carefully, answer your questions and look into what kind of support feels right for you.

Connecting the dots

Before we take the next step, we like to contact your Social Worker or Funding Authority to give them a heads up and make sure they’re part of the conversation.

We’ll keep you in the loop at every stage, so you always know what’s happening next.

Getting to know you

Next comes what’s often called an “assessment”, but we prefer to think of it as a relaxed and personal conversation.

It can take place wherever you feel most comfortable – in person, over the phone or on a video call.

This is our chance to really get to know you – not just your needs, but your likes and dislikes, your personality and what you want your future to look like. 

We’ll chat about things like:

  • What’s important to you day to day: your routines, interests and preferences
  • What kind of support helps you feel your best
  • When things feel tough, how can we make you feel safe and confident
  • The people, places and activities that make you happiest
  • Where you see yourself thriving (is it embracing the hustle and bustle of city life or retreating to the countryside or coast?)

It’s important to remember, there are no right or wrong answers. This is simply about understanding how we can support you to live the life you want to lead, in a way that feels just right.

Finding a place that feels like home

Now it’s back to us. We’ll jump straight into exploring options and working alongside Home Managers to find somewhere you could call home.

You’ll then be invited to visit these homes, see what life at Choice feels like, meet the team and the people who live there.

We want you, and those closest to you, to be able to imagine yourself living there: comfortable, supported and surrounded by people you trust.

Getting the green light

When you’ve found a place that feels like home, we’ll speak to your Social Worker or Funding Team and submit an offer letter that will be taken to panel for approval.

Our Referrals Team and your new Home Manager will be working to keep things moving and answer any questions you may have so you feel informed, involved and supported. Once we’ve got the ‘green light’ and funding has been approved, it’s all hands on deck to support you to move into your new home!

Moving in and making it yours

Moving into a new home is a big moment, full of excitement and sometimes a few nerves, but we’ll plan your transition carefully, at your pace.

You’ll have as much time as you need to get to know your surroundings, support team and even housemates! Our focus remains on helping you feel at home, confident and comfortable.

Of course, moving in is just the beginning of your next chapter. You’ll continue to be surrounded by people who truly believe in you. Because for us, this isn’t just about where you live – it’s about how you live and we’re here to support you to thrive.
