Supporting Sarah and Chloe to thrive

Sarah and Chloe are 28-year-old twins who live independently in Supported Living, with support from their team in the West Midlands. Both have learning disabilities associated with Chromosome 10 deficiency and – while they are able to communicate verbally – they benefit from emotional reassurance from the people they know and trust.

The twins moved to their new home in 2016 and since then the team have helped support them to achieve positive changes in their daily life.

The twins had previously experienced limited opportunities to make choices and try out new things. The team recognised the importance of supporting both Chloe and Sarah to live a life they want to live, with opportunities that reflected their interests.

How did we support Chloe and Sarah?

Although they enjoy spending time together, Sarah and Chloe have their own distinct personalities, interests and goals. Sarah enjoys arts and crafts, trips to the cinema and having her nails done, while Chloe loves playing games and spending time on her Xbox.

By using a more person-centred approach, providing a range of choices and supporting each sister to pursue her own interests, the team has helped them build confidence and take more control of their daily lives. Chloe and Sarah are also developing practical skills around household tasks, they enjoy cleaning and cooking and setting up plans for the rest of the week.

Celebrating outcomes and successes

Sarah and Chloe have made great progress and are now enjoying more independence and new opportunities. One of their biggest achievements was taking their first holiday abroad to Lanzarote, an experience they loved so much that they are already planning to return!

With their newfound confidence in making decisions and developing new skills, they continue to pursue hobbies and enjoy quality time together as sisters. Both have also embraced healthier lifestyles, leading to positive wellbeing outcomes. Sarah was overweight before and has been able to lose weight due to a more active and healthier lifestyle, encouraged by the team.

Sarah and Chloe feel they are brilliantly supported by the Supported Living and the Specialist Support Team at Choice Care, who work closely with them to achieve their goals.  The twins have progressed so much that they are now 1-1 support, whereas before they were 1-2, which further demonstrates their newly found independence.
