Supporting Kevin to regain stability and independence

Kevin lives at Dibden View and has mental health needs that can have a significant impact on his wellbeing and independence. Kevin also has a stoma, making it particularly important that his health is closely monitored.

Towards the end of 2024, when Kevin was going through a very stressful situation, the team at Dibden View noticed changes in Kevin’s wellbeing, including difficulties with sleep, increased impulsivity and challenges managing daily life. At the same time, Kevin was navigating important decisions about his future and needed additional support to maintain both his physical and emotional wellbeing.

Our approach

Through early intervention, colleagues at Dibden View acted quickly when they recognised that Kevin’s wellbeing was beginning to deteriorate.

Working closely with Kevin, his family and local mental health professionals, the team shared information, raised concerns and advocated for the support he needed. Colleagues carefully monitored changes in his wellbeing, recorded observations and worked collaboratively with professionals outside of Dibden View to ensure decisions were informed and person-centred.

Throughout this difficult time, colleagues made sure that maintaining Kevin’s dignity and independence remained at the heart of every decision. The team supported him to manage his medication safely, maintain his physical health and make choices about his future, while providing reassurance and consistency from people he knew and trusted.

Outcomes and successes

The combination of early intervention, personalised support and effective partnership working helped Kevin regain stability without requiring a hospital admission, which was an amazing outcome!

As his wellbeing improved, Kevin became more confident in recognising when he may need additional support and developed a greater understanding of how to maintain his mental wellbeing. He is now making positive decisions about his future, continuing to build his independence and managing his finances more successfully.

Kevin’s physical health remained stable throughout this period, and despite the challenges he faced, he avoided complications that could have impacted his overall wellbeing.

Today, Kevin is thriving and looking ahead with confidence. He is passionate about sharing his experiences to help others better understand mental health and the importance of seeking support when it is needed.

Chloe Randall, Registered Manager at Dibden View says:

“Watching Kevin go through a tough time was really hard but seeing how well he wanted to do and how much support he had around him was amazing, Kevin is one of the nicest people I have ever met and to be able to work with him has been an absolute pleasure.”

Amy McCall from Choice Care’s Specialist Support Team says

“Kevin’s determination throughout this difficult period did not go unnoticed. Despite facing significant challenges, he remained engaged with support and focused on achieving positive outcomes. This is an excellent example of effective collaboration between Kevin and the team at Dibden View, who responded early to concerns, worked proactively and ensured that Kevin remained at the centre of every decision. Their support enabled him to navigate a difficult period while maintaining his independence. It has been fantastic to see the progress Kevin has made and the confidence he has gained in managing his wellbeing, making positive decisions and looking ahead to the future.”

By working together and focusing on what mattered most to Kevin, the team helped him maintain his dignity, avoid a hospital admission and achieve positive, lasting outcomes.
