Community support is the term given to a package of support which is usually delivered in the community, rather than in the individual’s home, although of course support can be delivered in the home if that is what the individual would like.
Typically, those seeking a package of community support are looking for help to build on their social networks, gain employment or engage in community based activities. They may also want support to manage their finances or access health appointments. How of support are discussed with the individual and their funding authority / care manager and a personal budget is allocated. The individual, with or without the assistance of their care manager, will then choose a provider through a process on interviews.
