People with a learning disability can feel isolated from their community, struggle to communicate and manage social situations. This can hinder their independence and impact negatively on their self-esteem. To enable people with a learning disability, autism and other syndromes to live a meaningful and fulfilled life, we tailor our services specifically to each individual, no matter what their level of complexity, to become more independent, confident and live their life as a valued member of society.
To help the people we support progress along a positive care pathway that reflects their personal ambitions, likes and dislikes we offer person centred care plans with support provided by a highly trained, multi-disciplinary team including positive behaviour support.
Packages of care
Our care packages are available for those aged 16-plus through to older adulthood. We have a variety of homes throughout Wiltshire, catering for both younger and older individuals, delivered within a residential setting and across the wider Choice Care Group, within supportive living tenancies.
