Care Services Wiltshire
People with a learning disability can often feel isolated from their community, struggling to communicate and manage new social situations; this can hinder their independence and impact negatively on their self-esteem. To assist and enable individuals with a learning disability, autism and other syndromes to live a meaningful and fulfilled life, we tailor our social care services to support everyone, no matter what their level of complexity, to become more independent, confident and live their life as a valued member of society.
To successfully help the people we support progress along a positive care pathway that reflects their personal ambitions and desired outcomes, likes and dislikes we offer person centred care plans with support provided by a highly trained, multi-disciplinary team which includes positive behaviour support.
Packages of care
Our care packages are available for those aged 16-plus through to older adulthood and as such we have a variety of homes available throughout Wiltshire, catering for both younger and older individuals, delivered within a residential setting and across the wider Choice Care Group, within supportive living tenancies.
If you are looking for a care provider you can trust, who has an open and honest culture, strongly embedded Core Values and rigorous systems in place for governance and quality control, please read our website in more detail. Here you will find out more about us as an organisation and details of exactly how we work with individuals and their families / advocates to achieve positive outcomes. Alternatively, call us on 0203 195 0134 or email us at firstname.lastname@example.org and we will be happy to talk to you.
