People with a learning disability can often feel isolated from their community, struggling to communicate and manage new social situations; this can hinder their independence and impact negatively on their self-esteem. To assist and enable individuals with a learning disability, autism and other syndromes and medical conditions live a meaningful and fulfilled life, we at Choice Care Group tailor our social care services - which are available throughout Wiltshire - to support everyone, no matter what their level of complexity become more independent, confident and live their life as a valued member of society.
To successfully help the people we support progress along a positive care pathway that reflects their personal ambitions and desired outcomes, likes and dislikes we offer person centred care plans with support provided by a highly trained, multi-disciplinary team which includes in-house psychology and psychiatry.
Supported Living & Residential Care Homes
Our care packages are available for those aged 16-plus through to older adulthood and as such we have a variety of care homes available throughout Salisbury, Wooten Bassett and Swindon, Wiltshire, catering for both younger and older individuals, delivered within a residential setting and across the wider Choice Care Group, within supported living tenancies.
