Choice Care Group service users make successful transition to supported living

Two service users of specialist care provider Choice Care Group recently made a successful transition to a supported living placement.

The two service users made the transition from their residential placement at the Hazeldene care home in Reading into their own tenancy nearby, with support provided by Excel Support Services, a leading provider of supported living services to people with learning disabilities, which is now part of the Choice Care Group.

By joining up with Excel Support Services, Choice Care Group remain committed to helping people progress through a pathway of care, no matter what their personal difficulties.

Both of the service users have learning disabilities, autism and complex needs but have now gained more independence and responsibilities since moving to the supported living placement.

The two service users, ‘D’ and ‘H’, were recently interviewed about their transition to supported living.

D said, commenting about the move:  “I feel I’ve accomplished a lot in terms of my personal goals. I feel happy and confident, which the staff are happy to see, as we all put a lot of planning in to this move”.

H said the move was a success because of the support they received from the staff at Excel, and that being in charge of his own finances has meant that he can live more independently.

Edwina Johnston, chief executive officer of Choice Care Group, said: “This successful transition of the two service users shows how Choice Care Group has been able to extend its services for people with learning disabilities to a supported living environment.

“With the continued support from support workers and the management at Hazeldene and Excel, the two service users were able to feel confident enough to make the move and to become more independent and responsible in their everyday lives.”
